Make A Declaration

To begin the declaration process, send an email to requesting a Declaration Form along with your company name, contact information, the type of product, and the name of the product or service. You will receive specific instructions for completing and submitting additional information as the process continues.

Summary of the process:

  1. Review the following documents:
  2. Email and request the "OVAL Adoption Program Declaration Form."
  3. The OVAL Moderator emails you the declaration form.
  4. Complete the a copy of the form for each product that will support OVAL and return it to
  5. The form is reviewed by the OVAL Moderator and the organization’s product(s) or service(s) is posted with the organization’s Phase 1 information on the OVAL Adoption Program Participants page on the OVAL Web site.
